Re-plans and fences the exact reviewed revision and plan fingerprint, then resumes one operation-keyed reconciliation journal across ordinary Automation and PermissionAssignment mutations. A new webhook may return waiting_for_input without exposing or retaining a secret; Idempotency-Key is required.
