Reconcile a managed provider webhook registration

Inspect the provider hook and apply lost-response adoption: only the exact callback key plus config fingerprint may be claimed. Mismatch or unproved ownership settles degraded or indeterminate rather than duplicating. Delete converges when the provider already removed the hook.
