Update ExternalInstallation state

Moves an ExternalInstallation between active, disabled, disconnected, and uninstalled under a compare-and-swap on expected_version. Returning to active additionally requires a fresh signed pairing_proof naming the reactivate operation and this installation. Any non-active state stops every placement answering in that provider tenant.
